Addressing Health Disparities

Strategies, interventions and partnerships that lead to improved health outcomes and reduced health disparities

Healthcare organizations face difficult challenges in the world of health equity – from health disparities among patient populations to income inequality in the healthcare workforce to diversity in healthcare leadership. Solving equity means having the right values, making the right decisions and implementing those decisions. ProspHire will partner with you to identify and execute a plan that addresses the disparities in your unique communities.

Whether you’re a payor, provider or government entity, social determinants are a critical component of health care today and play a key role in addressing health equity. Social Determinants of Health (SDOH) are defined as “conditions in the environments in which people are born, live, learn, work, play, worship and age that affect a wide range of health, functioning and quality of life outcomes and risks”.

There are certain steps that can be taken to improve the conditions in which individuals live. The conditions often considered social determinants, or drivers, of health range from access to healthcare services to neighborhood and physical environment. Disease prevention and health promotion must not and cannot be confined to the walls of our care providers. ProspHire will partner with you to create an action plan that promotes health as worthy cause in which to invest.

At ProspHire, we seek to serve our communities, our clients and their customers and seek equitable health outcomes for all. The zip code you are born in should not immediately determine your health trajectory as it does today and our methodology seeks to drive this change through social determinants of health and an evidence-based approach.
